If Surgical Errors Are A "Never Event" Why Are Wrong Side Surgeries or Mistakes During Surgery Still Happening?
In Pennsylvania alone, according to Pennsylvania's Patient Safety Authority, more than five patients each month are victims of a surgical error. Information that was collected since 2004 showed that 416 wrong site surgeries have been performed statewide. That averages out to approximately 64 surgeries each year where there was medical negligence. Surgical mistakes or surgical accidents can be caused by a doctor, surgeon, anesthesiologist, certified nurse anesthetist (CRNA), physician assistant (PA) or nurse.
Due to the alarming rate of surgical errors, the Joint Commission, the organization that accredits hospitals, developed a "Universal Protocol" that requires every doctor, surgeon, nurse, nurse anesthetist, surgical technician or anyone else involved in the surgery to take a "time-out" to confirm the right surgical procedure and correct patient, in addition to completing a check-list prior to any surgery.
Any error in surgery can lead to the wrong part of the body being removed, the wrong surgery being performed, the wrong part of the body being operated on, the wrong side of the body being operated on, the wrong patient being operated on, medication error, nerve damage, anesthesia error, perforated bowel, nicked organs, or surgical instruments such as retractors, sponges, surgical clips or surgical towels being left inside the patient.
